Documentation

Anyone who has decided to participate in any physical activity will be required to complete both of the documents below prior to commencing a program with me.  There is a brief description as to what each form is and why completion is necessary.  

If you have any questions or concerns you should consult with your doctor or allied health professional and additionally contact me directly.

PAR-Q Document
PAR-Q

This form is a one page document that is called the 'Physical Activity Readiness Questionnaire.  It asks a series of questions related to your physical status to determine if you should consult with your family physician before starting a fitness program.

Keeping fitness safe
WAIVER OF LIABILITY

 

Although every effort is made to ensure the safety and well being of every personal training client and participant in group training sessions, there can rarely be unforseen circumstances that arise.

 

This document is designed to outline the potential risks of participation and allows the client to decide whether participation is for him/her.
